How a QR Code Reduced OSHA Recordables by 5x in 4 Years

This is a story that starts with a manufacturing facility with an OSHA log
full of injuries and a TRIR well above industry average and ends with
an 80% reduction in recordable injuries over the course of a four-year
span. One simple tool, (and its evolution), has led to site-wide hazard
recognition, management risk awareness and recurrence prevention
through the hierarchy of controls. We stopped focusing on the
recordable injuries metric, and measured other data points and the
